What Are Chronic Care Management Requirements?
At MHM we care that your needs and the attention you deserve are 100% covered. Chronic care management (CCM) focuses on serving individuals on Medicare with two or more chronic conditions. CCM is a preventative service, helping you if you are a Medicare patient to take a proactive approach to your health and wellness, while keeping you connected with your provider.
According to recent statistics, 80% of chronic diseases could be eliminated by preventative measures. Also, two-thirds of all deaths in America are attributable to one of five chronic disorders: cancer, chronic obstructive pulmonary disease, diabetes, heart disease, and stroke.
If you comply the following requirements, you can be part of our CCM program.
- You are a Medicare patient and have at least two chronic conditions
- Your conditions are expected to last at least 12 months
- You receive an individualized care plan
- Clinical time spent with you if you are enrolled must be at least 20 minutes and go up to 60 minutes
